Malladus, as he appears after losing the body he has possessed. Some of his design sketches have two horns, which are characteristic of his final form.

Malladus as he appears in his final form, having possessed Chancellor Cole and taken control over his body. In the final game, he appears as a 4-legged beast, but it seems that a humanoid form was designed for him as well.
